HDMI Crash with Intel Core i5-4210U

Hello, I'm currently trying to use a Monitor with my Hackintosh, but my Laptop Display would go black, and the computer would crash! I don't understand why this is happening. Computer Specs is given below

HP 15-r130nx
Intel Core i5-4210U
12 GB Ram (DDR3 - 1600 Mhz)
1 TB Sandisk SSD + 500 GB Sandisk SSD (Mac installed here)
Intel HD Graphics HD 4400 + Nvidia Geforce 820M (disabled)
Realtek Wireless
Realtek 8100 10/100 Ethernet

I have GPU Acceleration working fine + my Monitor & Laptop display are working via the Intel GPU

I know WIFI wouldn't be able to work, but currently, I'm using Ethernet, so it's not a big deal.
