Messed up resolution on install/setup boot on ThinkPad T430

Lenovo ThinkPad T430
CPU i5-3320M
GPU Intel HD Graphics 4000
8 GB ram

I've been following some tutorials on this and I've got to as far as getting the macOS utilities/setup to boot. It shows the Apple logo and loads for a bit, then the Apple logo goes to the right of the screen and gets cut in half, and the resolution for the macOS utilities page is messed up. What could be going on? FYI, in bios I have it set to UEFI only with no CSM support. SATA controller mode is AHCI (only other option is "compatibility")
