Help Moving to OpenCore

I currently have a Haswell hackintosh running OSX 10.14.6 with Clover.

I've been following the guide here https://dortania.github.io/OpenCore-Install-Guide/ to do a clean install on this machine with OpenCore and Monterey. I setup my EFI on a USB, but when I boot into it, it works the first time, then, on a reboot, it will always fail with "Failed to load configuration!". I also notice in this second boot it says the booter path is "EFI\OC\OpenCore.efi/\EFI\BOOT\BOOTX64.EFI" which looks wrong to me.

I can fix it by booting back to Clover going to the UEFI shell and running "bcfg boot rm 0". I see when I do a "bcfg boot dump" that the entry for OpenCore is: "HD(...omitted...)/EFI\OC\OpenCore.efi/\EFI\BOOT\BOOTX64.EFI".

So I'm wondering if OpenCore is adding some weird boot entry or my BIOS is appending that extra BOOTX64 part (with all the mixed forward/backward slashes).
